We welcome self-referrals as well as those from health care professionals. If you have questions about our programs or services, call 310.473-2911.
PAYMENT
Payment in full is required at the time services are rendered; we accept cash, check, Visa, Master card.
Many health services may be covered under your insurance plan; we will gladly provide you with an itemized bill to submit to your insurance company. Coverage varies widely by plan. Please check with your insurance carrier in advance of your appointment to determine which services will be covered, and if a referral from your primary care physician is required before scheduling services.
CANCELLATIONS
Due to the specialized nature of my services, cancellations must be received at least 24 hours in advance of the appointment time or payment in full is due. Please let us know as soon as possible if you will be unable to keep a scheduled appointment.
MERCHANDISE REFUND / RETURN POLICY
If you are not completely satisfied with any product within 30 days- we'll be happy to give you an exchange, credit, or refund, as you wish. Simple return your purchase to us, and please let us know why you were dissatisfied-it will help us to provide better products and services in the future. Shipping and handling fees are non-refundable.
MERCHANDISE SHIPPING POLICY
All equipment orders delivered in 2-4 weeks via UPS or whichever shipper is most time and cost effective to you.
